  1. Current Requirements for Certification in Schema Therapy (Individual)
  2. Checklist of documentation (with PDF document fields) or Checklist of documentation (with Word fields). Please email one of these forms along with your application to your regional coordinator. (In order to fill in and save the pdf forms you need a current and compatible PDF reader, for example Adobe Reader, google Chrome or Microsoft Edge.)

  11. Session summary form to send with recording to independent rater
